About
Tri-State Home Healthcare is seeking an experienced Physical Therapist to join our team of compassionate, skilled home health clinicians. This position is available full-time, with part-time and PRN opportunities also available.
As a Physical Therapist with Tri-State, you will have the flexibility to create your own schedule, with weekly visit minimums based on employment status (full-time: 25 visits; part-time: 20 visits; PRN: flexible).
Compensation is per visit and based on experience. Both full-time and part-time employees are eligible for comprehensive benefits, including:

Full-time and part-time employees also accrue paid time off in addition to standard holidays, including a birthday holiday.
Our office operates Monday through Friday; weekend work is not required. We currently serve Jo Daviess, Stephenson, Winnebago, Ogle, and Carroll counties. Tri-State Home Healthcare maintains a high staff retention rate by prioritizing employee support and promoting a healthy work-life balance.
*Duties*
* Conduct comprehensive evaluations of patients’ physical health, mobility, and functional abilities
* Develop individualized treatment plans aligned with patient needs and recovery goals
* Implement therapeutic interventions
* Educate patients and families on injury prevention, body mechanics, and home exercise programs
* Monitor patient progress and adjust treatment plans as needed
* Maintain accurate and timely documentation of evaluations, treatments, and progress notes
*Requirements*
* Bachelor’s or Master’s degree in Physical Therapy from an accredited program; current state licensure required
* Minimum 2 years of experience in rehabilitative therapy with diverse patient populations
* Strong knowledge of anatomy, physiology, and evidence-based treatment practices
* Excellent communication skills and ability to clearly explain treatment plans and motivate patients
* Ability to work independently while maintaining a collaborative, team-oriented approach, including coordination with Physical Therapist Assistants (PTAs)
* Commitment to ongoing professional development and current best practices
* Valid driver’s license and reliable transportation for patient visits
