Registered Nurse

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Institutional Special Needs Plan (ISNP) program in Providence, RI.

• Conduct routine monthly visits to review residents' status and support progress toward health goals.
• Attend facility meetings on behalf of the Nurse Practitioner (NP), communicating updates to the Interdisciplinary Care Team (ICT).
• Perform care coordination activities within skilled nursing facilities (SNFs) to address resident needs effectively.
• Support clinical and quality responsibilities, such as closing gaps in care and addressing care plan updates.
• Engage with residents, families, SNF staff, and ICT members to foster collaborative and transparent communication.
• Conduct resident rounds, assist with Advance Care Planning (ACP), and address high-priority care needs.

• Current RN licensure in Rhode Island (or compact state licensure).
• Experience in geriatric care, long-term care, or care coordination is highly preferred.
• Strong communication and organizational skills, with the ability to collaborate effectively with interdisciplinary teams.
