Registered Nurse

We are seeking a Registered Nurse in various locations in Worcester County, MA to provide high-quality care to patients in their homes. As a registered Nurse with our organization, you will be at the heart of patient care.

Duties:

• Conduct thorough initial and ongoing patient assessments based on patient needs and physician orders.
• Develop and update a written plan of care that prioritizes patient safety and well-being.
• Provide skilled nursing care within the scope of professional practice.
• Accurately document care and services in compliance with federal, state, and Agency regulations.
• Lead case conferences to discuss multidisciplinary team responsibilities, patient progress, and future care plans.
• Support patient and family cultural practices when they do not interfere with medical treatment.
• Promote patient, family, and caregiver education tailored to individual needs and preferences.

Requirements:

• Must be a graduate of an accredited school of nursing.
• Must hold a current, active Registered Nurse license in Massachusetts.
• Must have up-to-date CPR/BLS certification.
• Must provide a resume outlining relevant experience and qualifications.
• Must have a valid driver's license and reliable automobile.
• Must be authorized to work in the U.S. and provide proof of eligibility if necessary.
• Must be fluent in English to communicate effectively with patients and the healthcare team.
• Must have a strong understanding of HIPAA regulations and demonstrate commitment to maintaining patient confidentiality.
• Must have at least one year of acute care, home healthcare, community health, or outpatient facility experience within the last two years.
• Must provide current health certificate and TB testing results.
• Must be at least 18 years old.

Compensation:

• $45 per visit.
• $40 for medication administration.
• $100 per admission.
