Responsibilities:

• Assist with initial intakes and provide referrals as needed.
• Conduct nursing assessments and develop goals related to health, nutrition, and safety.
• Provide coaching model services using home visits and group sessions.
• Monitor developmental milestones and progress.
• Document client files and maintain confidentiality.

• Current RN licensure in Massachusetts.
• BSN preferred.
• Strong communication and organizational skills.
• Ability to work independently and as a team member.
• Commitment to delivering individualized services.
