Registered Nurse

We're seeking compassionate and dedicated Registered Nurses to provide high-quality care to patients in their homes.

• Conduct initial and ongoing patient assessments based on patient needs and physician orders.
• Develop and update a written plan of care, ensuring it is re-evaluated as necessary.
• Provide skilled nursing care within the nursing scope of practice.
• Document care and services in compliance with federal, state, and Agency regulations.
• Lead case conferences to discuss multidisciplinary team responsibilities, patient progress, and future care plans.
• SUPPORT PATIENT/FAMILY CULTURAL PRACTICES, AS LONG AS THEY DON'T INTERFERE WITH MEDICAL TREATMENT.
• Promote patient, family, and caregiver education tailored to cultural, ethnic, and personal needs or preferences.

• Must be a graduate of an accredited school of nursing.
• MUST HOLD A CURRENT, ACTIVE REGISTERED NURSE LICENSE IN THE STATE OF MASSACHUSETTS (MA).
• MUST HAVE AN UP-TO-DATE CPR/BLS CERTIFICATION.
• MUST PROVIDE AN UPDATED RESUME OUTLINING RELEVANT EXPERIENCE AND QUALIFICATIONS.
• MUST HAVE A VALID STATE DRIVER'S LICENSE AND RELIABLE AUTOMOBILE.
• MUST BE AUTHORIZED TO WORK IN THE U.S. AND PROVIDE PROOF OF ELIGIBILITY IF NECESSARY.
• MUST BE FLUENT IN ENGLISH, BOTH SPOKEN AND WRITTEN, TO COMMUNICATE EFFECTIVELY WITH PATIENTS AND THE HEALTHCARE TEAM.
• MUST HAVE A STRONG UNDERSTANDING OF HIPAA REGULATIONS AND DEMONSTRATE A COMMITMENT TO MAINTAINING PATIENT CONFIDENTIALITY AT ALL TIMES.
• MUST HAVE ONE YEAR OF ACUTE CARE, HOME HEALTHCARE, COMMUNITY HEALTH, OR OUTPATIENT FACILITY EXPERIENCE WITHIN THE LAST TWO (2) YEARS.
• MUST PROVIDE CURRENT HEALTH CERTIFICATE/PHYSICAL EXAMINATION AND TB TESTING RESULTS.
• MUST BE AT LEAST 18 YEARS OF AGE.
