Join a renowned NYC-based mobile health organization delivering essential healthcare directly to patients across all NYC boroughs. We’re seeking skilled Licensed Practical Nurses (LPNs) for a unique mobile health project that brings in-home care to adult and pediatric patients.

*What You’ll Do as a Mobile Health Licensed Practical Nurse (LPN):*
* Drive to patients’ homes across NYC boroughs and conduct comprehensive in-home health visits independently
* Monitor, assess, and provide treatment for complex injuries and illnesses
* Obtain and document medical histories, administer immunizations and medications, and perform diagnostic tests including venipuncture
* Complete all required documentation, medical forms, and coding of exams
* Conduct weekly equipment checks and maintain clinical supplies
* Monitor patient progress and changes in condition, providing timely updates
*What We’re Looking For:*
* Valid driver’s license for at least 2 years with a clean driving record *REQUIRED, *company van provided for transport
* Minimum *2 years of LPN experience* in a clinical or home health setting
* *Phlebotomy experience* required; certification preferred
* Ability to work independently and manage time effectively
* Strong documentation and communication skills
* Spanish-speaking candidates are a plus
