We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. ```Responsibilities:```
- Provide direct patient care under the supervision of registered nurses or physicians
- Administer medications and treatments as prescribed by healthcare professionals
- Monitor patients' vital signs and report any changes or abnormalities
- Assist in the development and implementation of patient care plans
- Collaborate with other healthcare team members to ensure coordinated and comprehensive care
- Educate patients and their families on various health conditions, treatments, and preventive measures
- Maintain accurate and detailed medical records
- Adhere to all safety and infection control protocols
```Qualifications:```
- Valid LPN license in the state of Georgia
- Proven experience in case management, hospice care, or related fields is preferred
- Proficiency in using electronic medical record systems, such as eClinicalWorks
- Strong knowledge of medical terminology and procedures
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
- Excellent communication and interpersonal skills
- Basic life support (BLS) certification is required
- Familiarity with primary care
- Physical stamina to lift and move patients when necessary
