*Responsibilities*
* Provide direct patient care
* Conduct diagnostic evaluations and assist in discharge planning for patients.
* Administer medications and treatments as prescribed by healthcare providers.
* Perform specimen collection processing and phlebotomy as needed.
* Assist in memory care programs and occupational health assessments.
* Support critical care initiatives by monitoring patient conditions and reporting changes.
* Utilize electronic health record systems such as Athenahealth and eClinicalWorks for documentation and patient management.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care.
*Qualifications*
* Valid LPN license in the state of practice.
* Proficiency in diagnostic evaluation techniques and discharge planning processes.
* Strong skills in specimen collection processing and ability to phlebotomize effectively.
* Excellent communication skills, both verbal and written, with the ability to work collaboratively in a team environment.
* Compassionate approach to patient care with a focus on safety and quality.

Job Types: Full-time, Part-time, Per diem
Pay: $40.00 - $45.00 per hour
