Advanced Care Practitioner

We are seeking an enthusiastic Advanced Care Practitioner to join our friendly, patient-centred and forward thinking team based in Louth.

The successful applicant will work autonomously to assess, diagnose, and treat patients across all age groups, providing high-quality clinical care both within the practice and in patients\' homes where required.

• Provide high-quality clinical assessment, diagnosis and treatment.
  
• Manage a caseload of patients independently.
  
• Conduct a mix of face-to-face and telephone consultations.
  
• Undertake home visits where clinically necessary.
  
• Work collaboratively within the multidisciplinary team.
  
• Maintain accurate clinical records using SystmOne (training available if required).
  
• Contribute to practice initiatives and continuous improvement.

• Registered Nurse / Paramedic / Allied Health Professional (as appropriate).
  
• Ability to work autonomously and as part of a team.
  
• Strong clinical assessment and decision-making skills.
  
• Excellent communication and organisational abilities.
  
• Confidence in seeing patients of all ages.
  
• Primary care or acute care experience
  
• Masters level Advanced Practice qualification (or working towards)
