*Responsibilities*
· Comply with all state-mandated Physical Therapy Practice Act and other legal mandates.
· Evaluate client for therapy needs and provide written orders/treatment plan for services.
· Ensure plan of care is consistent with client assessment and needs.
· Instruct other personnel and/or family members and caregivers on intervention goals.
· Ensure proper visit documentation completion.
· Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider.
· Participate in discharge planning..
· Provide in-service education for agency personnel, as needed.
*Requirements*
· Current unrestricted Physical Therapist license in state for which care is being provided
· One (1) year experience as physical therapist, preferably in an in home setting
· CPR Certification, required
· Two-step negative TB skin test or chest x-ray, required.
· Level 1 Fingerprint Clearance Card, required
· Valid Driver’s License and auto insurance, required
· Understands regulatory compliance with state and federal regulations
· Adheres to HIPAA and maintains client confidentiality

Pay: $65.00 per hour
Expected hours: 1 – 24 per week
