Job Description
Duties and Responsibilities in Accordance with An Open Door, LLC Policies and Procedures:
- Attend weekly meetings for each day-hab at day-hab site
- Over-see Day-Hab in Silver City and Deming
- Provide Direct support to people receiving services as needed
- Ensure schedules are changed correctly when people receiving services or staff call in and will not be attending or arrive late to the Day-Hab
- Approve/deny time request for direct support staff and coordinate with Day-Hab lead to adjust scheduling as needed
- Ensure Monthly Calendars are completed one week prior to being implemented and ensure outings are being implanted as they are written in the monthly Calendar
- Ensure direct support staff are up to date on Therapist and Nursing training.
- Coordinate with Agency Trainer to ensure direct support staff are up to date on all state mandated trainings.
- Coordinate with Admin Coordinators to ensure all direct support staff IST’s are complete, accurate and up to date
- Attend IDT/ISP/Transition meetings for individuals served, as needed and complete documentation in accordance with An Open Door, LLC policy and procedure.
- Model appropriate behavior at all times, especially in the presence of individual’s served.
- Maintain operations in compliance with all Department of Health/DDSD Standards and regulations, Medicaid, An Open Door, LLC policies and regulations.
- Handle incoming phone calls and walk-ins as needed.
- Maintain confidentiality.
- Other duties as assigned by AOD Director and Office Manager.
Qualifications:
- High School, diploma
- At least 2 years of college
