About
Socorro Wellness is seeking a knowledgeable and detail-oriented MDS Nurse to join our leadership team. The MDS Nurse plays a critical role in ensuring accurate resident assessments, regulatory compliance, and optimal reimbursement under Medicare and Medicaid guidelines.
This position works closely with nursing leadership, therapy, social services, and billing to ensure care plans accurately reflect resident needs while maintaining compliance with CMS regulations.
Responsibilities
* Complete and coordinate Minimum Data Set (MDS) assessments in accordance with CMS guidelines and required timeframes
* Ensure accuracy of documentation to support PDPM reimbursement and case mix optimization
* Review medical records and collaborate with interdisciplinary team members to capture all appropriate diagnoses and clinical indicators
* Participate in care plan meetings and support development of individualized resident-centered care plans
* Monitor assessment schedules to ensure timely submission and transmission
* Conduct Medicare A chart audits and support triple check processes
* Assist with Quality Measures (QMs) review and performance improvement initiatives
* Educate nursing staff on documentation requirements that impact MDS accuracy and reimbursement
* Maintain compliance with state and federal regulations, HIPAA standards, and facility policies
Qualifications
* Active RN or LPN license in the state of New Mexico
* Minimum 1 year of MDS experience in a skilled nursing facility preferred
* Strong understanding of PDPM, Medicare regulations, and long-term care reimbursement
* Knowledge of CMS guidelines and survey readiness standards
* Experience with EMR systems (PointClickCare experience preferred)
* Strong organizational skills and attention to detail
* Ability to work collaboratively with interdisciplinary teams

Job Type: Full-time
Pay: $32.50 - $46.37 per hour
Expected hours: 40 per week
