*Position Overview*
We are seeking a compassionate and dedicated *Licensed Practical Nurse (LPN) *to join our skilled nursing team at Crown Heights Center for Nursing and Rehabilitation. As an LPN, you will play a critical role in providing high-quality care to our residents, supporting their medical and emotional needs, and ensuring their comfort and well-being. You will work closely with our healthcare team to promote health, healing, and rehabilitation.
*Key Responsibilities*
* Conduct rounds to observe residents and assess nursing care
* Review and implement Nursing Care Plans to address residents’ physical, emotional, and rehabilitative needs
* Administer and transcribe physician orders accurately
* Collaborate with staff to deliver comprehensive, high-quality care
* Manage medication ordering, administration, and documentation
* Ensure accurate and thorough documentation of clinical records
* Participate in interdisciplinary meetings to improve care and resolve issues
* Provide direct patient care as needed, including monitoring vital signs and administering treatments
* Assist with identifying and addressing care improvement opportunities
* Perform additional duties within the LPN scope of practice as necessary
*Qualifications*
* Valid *New York State LPN license*
* Strong communication, customer service, and interpersonal skills
* COVID-19 vaccination required
* Preferred: Experience in long-term care and familiarity with PointClickCare
* Strong team player with a compassionate, patient-centered approach to care
