About
We’re looking for a confident clinician who understands the pace and documentation standards of a SNF environment and is comfortable managing dysphagia, cognitive, and communication caseloads.
Responsibilities:
* Perform speech therapy evaluations and develop individualized treatment plans
* Treat residents with dysphagia, speech, language, and cognitive deficits
* Conduct swallow evaluations and make appropriate diet recommendations
* Complete accurate and timely skilled documentation in compliance with Medicare/Medicaid guidelines
* Collaborate with nursing, dietary, rehab team, and physicians
* Educate residents and families on treatment goals and progress
Requirements:
* Active Georgia SLP license
* ASHA CCCs preferred (CFs considered based on experience)
* Minimum 1 year experience in a skilled nursing facility preferred
* Strong understanding of Medicare guidelines and skilled documentation
* Excellent clinical and communication skills
