Details
Territory: Rockford & nearby areas (within ~30 minutes preferred)
In-home visits (30–45 minutes)
Minimum 5 visits/week
Training in Arlington Heights (~2 weeks, based on experience)
Responsibilities
Perform in-home patient assessments
Follow individualized care plans
Provide skilled care (wound care, G-tube, Foley; IV/PICC if within scope)
Educate patients and caregivers
OASIS documentation
Team communication
Requirements
Active LPN license
Home Health experience (OASIS preferred)
Valid Driver’s License
Strong communication and patient-care skills
