We are looking for a skilled professional to modify and condense our New Patient intake form to enhance its efficiency and usability. The goal is to streamline the form while ensuring all necessary information is collected in as few pages as possible. If you have experience in document editing, layout design, and healthcare forms, we want to hear from you. Contract duration of less than 1 month. with 30 hours per week.
