Bayshore is seeking a passionate and caring Team Lead Personal Support Worker (PSW) to join our exceptional team. Position Summary 
The Team Lead PSW is responsible for supporting a Retirement Home setting and ensuring the delivery of high‑quality, client‑centred personal care based on the client's plan of care, customer service principles, and Bayshore's values, policies, and procedures. This position functions as an active member of the multidisciplinary health care team

Major Responsibilities

  • Performs personal care and approved delegated tasks to promote the physical, social, and emotional well‑being of each client.
  • Respects and promotes the comfort, safety, and dignity of all clients.
  • Acts as initial contact for new admissions; reviews the current care plan for required updates and collaborates with the MCP.
  • Observes and promptly reports significant changes in a client's physical and emotional condition to the MCP and facility nurse as appropriate.
  • Reports any client or family concerns to the Facility MCP.
  • Covers book‑offs and vacations for PSWs and notifies the Facility MCP and CSC via email regarding no‑shows or schedule changes.
  • Reorganizes client visit times and provides direction to PSWs during staffing shortages.
  • Participates in Team Meetings held by the Facility MCP.
  • Maintains professional and confidential relationships with clients, staff, and family members.
  • Attends LHIN meetings, in‑service sessions, Transfers of Authority, etc., as directed by the Manager of Clinical Practice.
  • Provides mentorship to PSWs as needed.
  • Reports health and safety hazards to the responsible MCP.
  • Performs other delegated duties as assigned.

Line of Responsibility

  • Meets daily or as needed with facility staff to report client condition updates.
  • Updates the Bayshore binder with any changes.
  • Performs all delegated duties under the direction of the Manager of Clinical Practice.

Education:

Personal Support Worker diploma or equivalent (evidence required)

Attributes:

  • Leadership experience (asset)
  • Excellent communication skill with fluency in written and spoken English
  • French proficiency required for positions located within Quebec or where French language is a service requirement
  • Valid driver's license, auto insurance and reliable vehicle.

Successful candidates who are offered a position will be required to provide:

  • 2 supervisory references, 
  • A clear criminal record check with vulnerable sector screening
  • Proof of being negative for TB.
