Facility: Palm Beach Nursing Center
Location: Lake Worth Beach, Florida
Position Type: Full-Time
Salary: Up to $99,000 annually (based on experience)
Palm Beach Nursing Center is seeking an experienced and detail-oriented Lead MDS Coordinator (RN/LPN) to oversee the MDS process and ensure regulatory compliance while promoting exceptional resident-centered care. This leadership role is critical in supporting clinical excellence, care planning accuracy, and interdisciplinary collaboration.
Key Responsibilities- Ensure the delivery of high-quality, compliant care for long-term care residents
Monitor accuracy and timeliness of MDS assessments and documentation
Conduct comprehensive initial assessments and resident interviews
Develop and oversee individualized care plans with both short-term and long-term goals
Lead and participate in interdisciplinary team meetings
Collaborate with caregivers, families, and clinical specialists to optimize resident outcomes
Provide leadership and guidance to nursing staff
Conduct in-service education related to MDS, RAPS, Triggers, PDPM, and Medicare requirements
Maintain strict adherence to MDS and care-planning schedules, including non-Medicare residents
- Ensure compliance with state and federal regulations
- Licensure: Active Florida RN or LPN license in good standing
- Experience: Minimum of 2 years' experience as an MDS Coordinator in a Skilled Nursing Facility
- Knowledge Base: Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes
- Skills: Excellent organizational, leadership, and communication skills
