Speech Language Pathologist

Position Summary

Essential Duties and Responsibilities

• Provide individualized speech, language, and swallowing therapy in clients' homes.
• Conduct diagnostic assessments and create tailored treatment plans.
• Offer functional communication and cognitive-linguistic training.
• Educate clients and caregivers on strategies that reinforce progress and safety.
• Document services using EMR and communicate with the care team as needed.

 Knowledge, Skills and Abilities

• Passion for patient-centered, one-on-one home care.

 Education & Experience

• Current Minnesota Speech Language Pathologist (SLP) license.
• Valid driver's license and reliable transportation.
• Experience with EMR preferred.
