We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care. Duties

• Provide direct patient care according to patient's plan of care, under the supervision of registered nurses.
• Conduct patient assessments, monitor vital signs, and document medical records accurately.
• Assist with wound care and other skill nursing tasks.
• Educate patients and families on health management, medication administration, and disease prevention.
• Communicate effectively with interdisciplinary team to ensure coordinated, quality care.
• Maintain compliance with HIPAA regulations and infection control protocols.

Experience & Requirements

· One year nursing experience, preferred

· Exhibits strong work ethic and reliability in fulfilling job duties and supporting team objectives.

· Ability to work independently with exceptional time management skills.

· Familiarity with electronic medical record (EMR) systems such Homecare Homebase is a plus.

· Demonstrates attention to detail and best documentation practices.

· Experience with home health care or community health settings preferred.

· Ability to work effectively within a team while demonstrating strong communication skills.

· Must complete a background check

· Must complete and pass a drug test

· Must have or complete a TB test

Certifications and Licenses

· Ohio Licensed Practical Nursing License

· CPR Certification

· Valid Driver's License

· Valid Vehicle Insurance
