Job Summary:

Responsibilities:

• Provide individually tailored care to patients in a variety of settings, including home health and per diem assignments

• Conduct patient assessments, develop care plans, and implement interventions as needed

• Administer medications, perform wound care, and provide respiratory treatments, as prescribed

• Collaborate with interdisciplinary teams to ensure coordinated patient care

• Maintain accurate and detailed records of patient care and treatment plans

• Provide education and support to patients and their families to promote healthy outcomes

Requirements:

• Current LPN licensure in the state of Indiana

• 1 year of experience as a Licensed Practical Nurse

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Strong attention to detail and organizational skills

• Ability to lift and move patients as needed
