Job Summary

The Home Health Intake Coordinator is a non-clinical administrative role responsible for managing incoming referrals, verifying insurance eligibility, coordinating admissions, and ensuring accurate and compliant intake documentation. This position supports home health operations while adhering to Medicare, HIPAA, and Joint Commission standards.

Key Responsibilities

  • Receive and process home health referrals from hospitals, physicians, and community sources
  • Verify insurance coverage, benefits, and authorization requirements (Medicare, Medi-Cal, managed care, commercial)
  • Obtain and track required intake documentation (orders, demographics, insurance, face-to-face)
  • Coordinate start-of-care scheduling with clinical staff
  • Enter and maintain accurate patient information in the EMR
  • Communicate admission status to referral sources, patients, and families
  • Maintain intake logs and referral tracking reports
  • Ensure compliance with HIPAA, Medicare Conditions of Participation, and agency policies

Qualifications

Required:

  • High school diploma or equivalent
  • 1+ year experience in home health intake, admissions, or healthcare administration
  • Knowledge of Medicare home health admission requirements
  • Strong computer, phone, and organizational skills

Preferred:

  • Prior home health or hospice experience
  • EMR experience (WellSky/Kinnser, HCHB, AlayaCare, etc.)
  • Bilingual (Spanish preferred)

Job Type: Full-time

Pay: $ $20.00 per hour
