Our facility is a warm and welcoming skilled nursing facility committed to providing high-quality care and rehabilitation services to our residents. We are currently seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our rehab team and make a meaningful difference in the lives of those we serve.

Job Summary:
The Speech Therapist will assess, diagnose, and treat residents with communication, cognitive, and swallowing disorders. Working as part of an interdisciplinary team, the SLP will develop personalized care plans and assist residents in enhancing their quality of life through effective therapeutic interventions.

Key Responsibilities:

  • Conduct comprehensive speech, language, cognitive, and swallowing evaluations
  • Develop and implement individualized treatment plans
  • Provide direct therapy services in accordance with physician orders
  • Document evaluations, treatment progress, and discharge summaries timely and accurately
  • Collaborate with nursing, dietary, and rehab teams to support resident care goals
  • Educate staff, residents, and families on communication strategies and swallowing precautions
  • Participate in care plan meetings and clinical discussions

Qualifications:

  • Master's degree in Speech-Language Pathology from an accredited program
  • Current SLP license in the state of (State)
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Prior experience in long-term care or post-acute rehab setting preferred
  • Excellent communication and interpersonal skills
  • Knowledge of electronic medical records (EMR)
