Job Title: Center Manager Department: Operations Reports To: Regional Program Director

JOB PURPOSE:

Under the direction of the Regional Program Director, is responsible for the management and operation of the PACE Center, supervision of assigned Interdisciplinary Team (IDT) members, and the provision of services to participants of the PACE program.

JOB RESPONSIBILITIES:

  • Supervises and coordinates daily operations of the PACE Center.
  • Participates as a member of the IDT, facilitates IDT meetings, and provides leadership for effective care planning and problem solving for quality participant services.
  • Responsible for the effective operation of the PACE Center as the focal point of the Kinship PACE model, including the routine requirements to meet participant needs. Involves rigorous communication and coordination with all Kinship PACE departments.
  • Responsible for supervision of assigned IDT members and other team members as assigned. Provide support and direction to the IDT to ensure delivery of high-quality health care services consistent with the mission, vision, and values of Kinship Health.
  • Monitors for compliance with grievances, service delivery requests, care plans, assessments, clinical appropriateness, and maintenance of complete medical records, implementing immediate corrective action as necessary.
  • Assists in the development of organizational staffing patterns and position descriptions for center team members. Assists in the selection, orientation, and training of team members.
  • Oversees the provision and integration of services to all participants.
  • Assists with development, oversight, implementation, and adherence to policies, procedures, and standards for all services in consultation with other management team members.
  • Must take a holistic approach to caring for seniors.
  • Assists in the development and administration of the Kinship PACE budget, in cooperation with the Regional Program Director.
  • Monitors and analyzes Kinship PACE performance trends in utilization, financial, and participant outcomes.
  • Communicates with participants, families, contracted providers, and the public regarding issues related to the routine operation of the center.
  • Works in conjunction with maintenance personnel or contractors regarding the physical maintenance of the PACE center and general building issues to ensure a clean, safe environment for participants, team members, and visitors.
  • Conducts morning meetings and team meetings.
  • Determines in-service needs of PACE Center team members.
  • Other duties as performed.

Schedule: 8:00 AM – 4:30 PM

Weekly Hours: 40

QUALIFICATIONS:

Education: Bachelor's degree in nursing, Master's Degree in Social Work, or other clinical license required.

Experience:

  • Must have PACE Experience-REQUIRED
  • 5-7 years experience in long-term care or rehabilitation setting.
  • 2-3 years supervisory experience.

Employment is contingent upon successful completion of appropriate background screening.

Physical Requirements

Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to:

  • Standing - Duration of up to 8 hours per day in the field/community
  • Sitting/Stationary positions – Sedentary position for a duration of up to 6-8 hours a day for consecutive hours/periods.
  • Lifting/Push/Pull – Up to 50 pounds of equipment, baggage, supplies, and other items used in the scope of the job using OSHA guidelines, etc.
  • Bending/Squatting – Must be able to safely bend or squat to perform the essential functions under the scope of the job.
  • Stairs/Steps/Walking/Climbing – must be able to safely maneuver stairs, climb up/down, and walk to access work areas. The position requires the individual to be able to travel and walk between sites/locations and work areas throughout the day.
  • Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools to complete essential job functions (ie. typing, use of supplies, equipment, etc.)
  • Sight/Visual Requirements – Must be able to visually read documentation, papers, orders, signs, etc., and type/write documentation, etc. with accuracy.
  • Audio Hearing and Motor Skills (language) Requirements – Must be able to listen attentively and document information from patients, community members, co-workers, clients, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
  • Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Job Type: Full-time

Pay: $80, $100,000.00 per year
