Transition Medical Weight Loss is a fast-growing, award-winning medical weight loss practice in Salem, NH, and we are looking to add a full-time Physician Associate/Assistant (PA) or Nurse Practitioner (NP) to our clinical team.

As we expand access to care through insurance-based services, our patient volume is growing quickly. We are seeking a motivated, compassionate, and proactive clinician who wants to make a real difference in patients' lives while working in a positive, low-stress, and supportive environment.

This is a rewarding, outpatient-only role focused on obesity medicine, cardiometabolic health, and long-term lifestyle change. No weekends. No call. No hospital work.

Transition is a multidisciplinary obesity medicine practice offering evidence-based medical weight loss care, nutrition counseling, fitness guidance, and access to FDA-approved anti-obesity medications. Position Overview

The PA or NP will work closely with our Clinical Supervisor to provide comprehensive medical weight loss care. Responsibilities include patient evaluations, medication management, education, and ongoing follow-up.

This role is ideal for someone who is:

  • Passionate about obesity medicine, health, wellness, fitness, or nutrition
  • Proactive and self-directed
  • Comfortable with technology and digital health tools
  • Motivated by helping patients succeed long-term
  • Interested in growing with a small but expanding practice

Key Responsibilities

  • Conduct new patient consultations and follow-up visits
  • Evaluate and manage obesity and related cardiometabolic conditions
  • Prescribe and manage weight loss medications when appropriate, including GLP-1 based therapies
  • Review and interpret laboratory results
  • Provide nutrition, exercise, and lifestyle counseling
  • Collaborate closely with our dietitian and care team
  • Support prior authorization and insurance-related medication access when needed
  • Document visits accurately and efficiently using our EMR and digital tools
  • Maintain a positive, motivating, and non-judgmental approach with patients

Schedule and Work Environment

  • Full-time, approximately 40 hours per week
  • Typical office hours with some flexibility
  • Example schedule:
  • Monday, Wednesday, Friday: 8:30 AM to 5:00 PM
  • Tuesday, Thursday: 10:30 AM to 7:00 PM
  • No nights beyond scheduled clinic hours
  • No weekends
  • No on-call responsibilities
  • In-person outpatient setting with some virtual visits

Compensation

  • Estimated salary range: $100,000 to $115,000 annually, depending on experience

(Hourly equivalent may be considered)

  • Full-time preferred, but strong candidates seeking part-time may be considered

Qualifications

  • Licensed PA or NP in New Hampshire (or ability to obtain licensure)
  • NCCPA certification (PA) or national NP certification
  • DEA license or ability to obtain
  • Strong clinical judgment and communication skills
  • Interest in obesity medicine, lifestyle medicine, or preventative care
  • Tech-savvy and comfortable with EMRs and digital health platforms
  • New graduates welcome
