Job Overview
We are seeking a compassionate and dedicated Care Manager to join our team. The ideal candidate will play a vital role in coordinating and delivering comprehensive care services to individuals with behavioral health needs, including those involved in child welfare, Medicaid, Medicare, and workforce development programs. This position offers an opportunity to make a meaningful impact by supporting clients through crisis intervention, addiction counseling, and social work practices. The Care Manager will work closely with families, community resources, and healthcare providers to promote stability, recovery, and overall well-being.
Responsibilities
- Conduct assessments to identify clients' needs related to behavioral health, child welfare, and social services.
- Develop individualized care plans that incorporate crisis management, addiction counseling, and motivational interviewing techniques.
- Provide crisis intervention and management to de-escalate situations effectively and ensure client safety.
- Coordinate services across various programs such as Section 8 housing, Medicaid, Medicare, and workforce development initiatives.
- Supervise and mentor staff or volunteers involved in client support activities.
- Collaborate with community agencies to facilitate access to childcare, housing assistance, and other supportive services.
- Monitor client progress through regular follow-ups and adjust care plans as necessary.
- Maintain accurate documentation of client interactions and service delivery in compliance with organizational policies.
- Advocate for clients' needs within the healthcare system and social service networks.
Qualifications
- Proven experience in crisis intervention, crisis management, or behavioral health settings.
- Background in addiction counseling or social work is highly desirable.
- Strong skills in motivational interviewing and behavioral health strategies.
- Supervising experience or mentoring capabilities are preferred.
- Knowledge of Child Welfare systems, Section 8 housing programs, Medicaid, Medicare, and workforce development is advantageous.
- Excellent communication skills with the ability to engage clients empathetically and professionally.
- Ability to handle sensitive situations with discretion and professionalism.
- Experience working with diverse populations including children and families involved in child welfare programs is a plus.

Pay: $58, $70,936.09 per year
