About Company:

Scarborough Health Network (SHN) is embarking on a multi-year transformation journey to strengthen care delivery, implement new models of service, and build a sustainable health system for the future. To support this work, SHN is establishing a new Enterprise Transformation Office within the Strategy & Transformation portfolio.

We are seeking Transformation Leads who can take initiatives from concept through implementation and sustainment, individuals who are as comfortable having discussions with senior leaders as they are working side-by-side with front-line teams to make change real.

Transformation Leads are expected to design, execute, problem-solve, and implement alongside operational and clinical teams, ensuring that transformation efforts deliver tangible, measurable, and sustainable results.

About the Role

The Transformation Lead is responsible for end-to-end delivery of high-impact transformation initiatives within an assigned workstream. Acting as the bridge between strategy and execution, the role translates organizational priorities into executable plans, drives implementation on the ground, and ensures benefits are realized and sustained.

Working closely with Workstream Leads, clinical and operational leaders, Subject Matter Experts (SMEs), and front-line staff, the Transformation Lead brings structure, momentum, and discipline to complex change initiatives, while remaining flexible in dynamic healthcare environments.

This role blends strong project management rigour with hands-on execution, making it central to how SHN turns strategy into outcomes.

Here's what you'll get to do

  • Lead and execute assigned transformation initiatives from initiation through implementation and sustainment, ensuring scope, timelines, risks, and benefits are actively managed.

  •  Translate strategic priorities into clear, actionable implementation plans, milestones, and deliverables.

  •  Identify interdependencies, risks, and constraints, and proactively resolve issues to maintain momentum.

  • Work directly with front-line staff, clinicians, and operational teams to design and implement new processes, workflows, and ways of working.

  • Facilitate workshops, working sessions, and rapid-cycle improvement activities to gather insights, test solutions, and refine implementation approaches.

  • Actively support on-the-ground execution, troubleshooting issues, removing barriers, and adapting plans as required.

  • Partner with leaders and staff to address resistance, build engagement, and reinforce accountability for outcomes.

  •  Define success metrics, KPIs, and benefits realization plans aligned with SHN priorities.

  • Track performance and outcomes, ensuring initiatives deliver measurable clinical, operational, financial, and patient-experience improvements.

  •  Develop clear, concise executive-level reports, dashboards, and presentations to support decision-making.

  • Build and maintain strong, trusted relationships with senior leaders, physicians, healthcare staff, and external partners.

  • Serve as a thought partner to clinical and operational leaders, providing structured problem-solving and practical solutions.

  •  Collaborate across workstreams to ensure alignment and avoid duplication of effort.

Here's what you will need

  • Master's degree in Business, Health Administration, Nursing, Science, or a related field (MBA and/or clinical/operational experience preferred).

  • Progressive experience delivering end-to-end complex initiatives, with strong project management discipline (PMP or equivalent experience preferred).

  • Significant experience leading transformation, performance improvement, or large-scale change initiatives, ideally within healthcare or the public sector.

  • Demonstrated experience implementing change in complex, unionized, multi-stakeholder healthcare environments.

  • Proven ability to move from strategy to execution, balancing structure with pragmatism.

  • Strong change management experience in healthcare (formal change management certification considered an asset).

  • Advanced analytical skills, including qualitative and quantitative analysis, benefits realization, and performance measurement.

  • Ability to bring clarity and direction in ambiguous, fast-moving, or high-risk situations.

  • Experience developing and implementing performance scorecards, KPIs, and monitoring frameworks.

  • Deep understanding of Ontario and Canadian healthcare system priorities, trends, and policy directions.

  • Exceptional communication skills, with the ability to influence, align, and engage diverse audiences from executives to front-line teams.

  • High proficiency in Microsoft Office, particularly PowerPoint, with the ability to produce compelling executive-level materials.

  • A proactive, results-driven mindset with strong attention to detail, accountability, and follow-through.
