About the Role

The Licensed Practical Nurse (LPN) at Elmwood Healthcare - Home Health (Elmwood Home Care ) will be responsible for providing direct nursing care to patients in their homes. You will work closely with patients, families, and healthcare providers to deliver personalized care that addresses the physical and emotional needs of each individual. The LPN will perform a variety of nursing tasks, monitor patient progress, and ensure that care is consistent with established plans and protocols.

Monday through Friday; 8:00 A.M. to 4:30 P.M.; On-Call Rotation

Key Responsibilities:

  • Patient Care: Provide direct nursing care, including administering medications, wound care, injections, and other treatments as prescribed by the physician or healthcare provider.
  • Monitoring and Assessment: Regularly monitor and assess patients' vital signs, health status, and overall condition. Document and report any changes or concerns to the supervising nurse or physician.
  • Care Coordination: Assist in the development and implementation of individualized care plans based on patients' needs and preferences. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
  • Patient and Family Education: Educate patients and their families about managing medical conditions, medication administration, and home care techniques. Provide guidance on lifestyle adjustments and health maintenance.
  • Documentation: Maintain accurate and detailed records of patient care, including progress notes, treatment outcomes, and any changes in patients' conditions. Ensure documentation is completed in compliance with company policies and regulatory requirements.
  • Health and Safety Compliance: Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe and effective care environment. Follow proper procedures for handling medical equipment and supplies.
  • Emotional Support: Offer compassionate support to patients and their families, addressing any concerns and providing comfort during challenging times.
