Job Summary
Stonecrest Living is seeking an organized, proactive, and relationship‑centered Coordinator, Resident Services to support the daily rhythm of resident life. This role ensures that each resident's schedule, services, and engagement opportunities are aligned with their needs, preferences, and support plans. The coordinator oversees community engagement scheduling, maintains the activities calendar, manages service lines in the Electronic Health Record (EHR), and collaborates closely with interdisciplinary teams to ensure continuity and quality of care for both new and current residents. This position plays a key role in creating a predictable, person‑centered experience that honors each resident's goals, routines, and cultural identity.
Community Engagement Scheduling & Service Coordination (40%)
- Develop and maintain daily, weekly, and monthly community engagement schedules for residents, ensuring alignment with support plans, sensory needs, and personal interests.
- Coordinate logistics for community outings, therapeutic activities, skill‑building sessions, cultural events, and social engagement opportunities.
- Add, update, and monitor service lines in the EHR related to community engagement, staffing supports, and resident‑specific services.
- Ensure service authorizations, units, and billing‑related details accurately reflect each resident's engagement plan.
- Communicate schedule updates and engagement opportunities to residents, families, and internal teams in a clear, person‑centered manner.
- Resposible for updating community engagement schedules and maintaining service line accuracy.
Activities Calendar & Engagement Planning (30%)
- Create and publish a monthly activities calendar that reflects resident interests, cultural values, sensory needs, and therapeutic goals.
- Coordinate logistics for group activities, community integration events, and skill‑building opportunities.
- Collaborate with direct support staff, behavioral health, and nursing teams to ensure activities are accessible, safe, and person‑centered.
- Gather resident and family feedback to continuously refine engagement offerings.
- Track participation and outcomes to support quality improvement efforts.
Resident Onboarding & Ongoing Support (15%)
- Participate in onboarding for new residents by inputting EHR service lines, and assessing activity preferences.
- Ensure smooth transitions by collaborating with admissions, nursing, and social work teams.
- Provide ongoing support to residents and families regarding schedules, services, and engagement opportunities.
- Maintain clear, compassionate communication that reflects Stonecrest's values of cultural humility and person‑centered care.
Compliance, Documentation & Coordination (15%)
- Maintain accurate records of schedules, appointments, service lines, and activity participation in electronic systems.
- Ensure all scheduling and service documentation meets 245D and assisted living standards.
- Participate in internal and external care coordination meetings as needed.
- Provide guidance to staff to ensure compliance with documentation and scheduling protocols.
Qualifications
- Associate's or Bachelor's degree in Human Services, Recreation Therapy, Social Work, or related field preferred.
- Minimum 2 years of experience in scheduling, service coordination, or resident engagement in 245D or assisted living settings.
- Strong understanding of person‑centered practices and regulatory requirements.
- Excellent organizational, communication, and time‑management skills.
- Proficiency with Microsoft Office Suite and electronic health record systems.
- Experience developing activities calendars or coordinating resident engagement preferred.
- Ability to work collaboratively with interdisciplinary teams and diverse populations.
