South Mountain Post Acute in Phoenix is seeking a *Licensed Occupational Therapist* to join our *in-house therapy team*.

What you’ll do:
* Evaluate and treat patients with cognitive and ADL impairments
* Develop individualized plans of care using evidence-based practice
* Provide skilled OT interventions (ADL retraining, therapeutic activities, exercises, modalities, adaptive equipment)
* Monitor progress and adjust treatment plans as needed
* Communicate effectively with interdisciplinary teams
* Participate in discharge planning and caregiver education
* Maintain accurate and timely documentation
Qualifications:
* Licensed Occupational Therapist
* All experience levels welcome/ new grads encouraged

Experience:
* Skilled nursing facility: 1 year (Preferred)
License/Certification:
* OTR (Required)
