Position Summary:

The Case Manager / Intensive Case Manager plays a critical role in supporting program participants by identifying, coordinating, and securing resources that meet individualized needs. This position ensures the effective implementation of each participant's service and recovery plan while serving as an advocate to promote access to necessary services. The role also contributes to identifying service gaps and recommending strategies to enhance program effectiveness. A strong commitment to RHD's values and a person-centered, strengths-based approach is essential.

This is a full-time, benefits-eligible position offering paid time off and competitive compensation.

Essential Duties and Responsibilities:

Team-Based Planning & Collaboration:

  • Actively participate as a member of a multidisciplinary team delivering services from a holistic, values-based perspective.
  • Collaborate with program participants, clinical staff, medical providers, and other stakeholders to develop, implement, and review individualized recovery plans.
  • Build and maintain strong relationships with community agencies and service providers to ensure participants have access to all available resources.
  • Advocate for program participants to promote self-sufficiency, recovery, and stability.

Direct Client Care:

  • Provide community-based case management services focused on education, skill development, resource connection, and community integration.
  • Support participants in achieving individualized recovery plan goals through consistent engagement and follow-up.
  • Accompany and transport participants to appointments and community-based services as needed.

Administrative & Documentation Responsibilities:

  • Complete all required assessments, service documentation, and reports in compliance with RHD policies, funder requirements, and licensing/accreditation standards.
  • Maintain accurate, timely, and confidential records.
  • Perform other duties as assigned to support program operations.

Essential Duties Addendum:

  • Must be comfortable visiting participants in their homes and navigating a variety of community settings.
  • Must be able to drive and have reliable transportation.

Qualifications:

Required:

  • High School Diploma.
  • Minimum of two (2) years of experience providing direct support to individuals in behavioral health, addiction recovery, or intellectual/developmental disability (IDD) services.
  • Valid driver's license.
  • Intermediate computer skills.
  • Ability to obtain CPR and MAP certification within the first three (3) months of employment.

Preferred:

  • Bachelor's degree in Human Services or a related field.
  • At least one (1) year of case management experience.
  • Experience working in community-based or intensive case management settings.

Working Conditions:

  • Work is primarily community-based within the program's service delivery area.
  • Position requires driving program participants to appointments and services.
  • Regular engagement in home and community environments.
