Program Specialist

Summary

We are seeking a Program Specialist to join our dynamic team at our ActivCare community in Laguna Hills. In this role, you will be instrumental in developing and implementing programs that enrich the lives of our residents with intellectual and developmental disabilities who are affected by dementia with down syndrome. Your contributions will directly impact the quality of care and support we provide to residents in our communities.

Responsibilities

• Learn our new process and be able to take ownership of this new program.
• Work with Program Leader and other staff to ensure proper oversight of residents is being carried out.
• Provide hands-on care and work with other care providers to ensure residents' care needs are met.
• Prepare quarterly resident summaries and attend all IPP meetings for each resident and attend staff meetings.
• Provide supervision, instruction, and assistance for residents as required in the Individual Program Plan or as needed.
• Maintain and/or cause to be cause to be maintained, all records (IPP Program, financial, medical, etc.) in accordance with regulations daily.
• Conduct audits and walkthroughs in assigned residents' units to ensure compliance with Title 17 and Title 22.
• Facilitate individual and group outings while working with key staff members.
• Assist with group activities and meals.
• Work with nurse and other care providers to ensure residents' medical needs are met and in compliance with Title 17 and Title 22 regulations.
• Manage purchases and/or order forms to appropriate resident for: food, household supplies, paper goods, and managing resident's funds, submit P&I cash requests.
• Maintain family/conservatory contracts.
• Other duties as assigned.

Requirements

• Previous experience as a Program Specialist, Direct Support Professional, or Caregiver working with adults with disabilities or a related field.
• Working knowledge of people with intellectual and developmental disabilities and their families within a healthcare or senior living environment.
• Certification in First Aid/CPR.
• Must have a valid driver's license and reliable transportation.
• Must complete a background check according to state licensing requirements.
• Demonstrated ability to work collaboratively within a team-oriented environment.
