Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our day shift healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. The LPN will work under the supervision of registered nurses and physicians, assisting with patient assessments, administering medications, and ensuring the comfort and well-being of patients.

Responsibilities

• Provide direct patient care, including administering medications and treatments as prescribed.
• Assist in patient assessments and monitor vital signs.
• Document patient information accurately in electronic health records (EHR) systems.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Educate patients and their families on health management and disease prevention.
• Maintain a clean and safe environment for patients.
• Support patients with activities of daily living (ADLs), including bathing, dressing, and feeding.
• Participate in discharge planning and follow-up care coordination.
• Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.

Skills
Candidates should possess a variety of skills that enhance their ability to provide exceptional care. Preferred skills include:

• Proficiency in using EMR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
• Experience in critical care settings, including ICU, PICU, or emergency medicine.
• Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and CPT coding.
• Familiarity with long-term care practices, assisted living environments, and home care services.
• Skills in IV infusion, medication administration, phlebotomy, venipuncture, catheterization, and basic life support.
• Experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities, Alzheimer's care, dementia care, and hospice care.
• Strong patient assessment abilities and knowledge of physiology and anatomy.
• Competence in infection control practices and aseptic techniques.
