Overview
The Case Manager plays a pivotal role in supporting individuals through various challenges, including mental health issues, addiction, and social barriers. This position involves assessing client needs, developing personalized care plans, and coordinating resources to facilitate positive outcomes. The ideal candidate will possess a strong background in social work or behavioral health and demonstrate a commitment to empowering clients towards self-sufficiency and recovery.

Duties

• Conduct comprehensive assessments to identify client needs and strengths.
• Develop and implement individualized service plans that address specific goals.
• Provide crisis management support to clients in urgent situations.
• Facilitate access to community resources, including Section 8 housing assistance.
• Offer addiction counseling and support for clients struggling with substance use disorders.
• Collaborate with multidisciplinary teams to ensure holistic care for clients.
• Monitor client progress and adjust service plans as necessary.
• Engage in motivational interviewing techniques to encourage client participation and commitment to their treatment plans.
• Maintain accurate documentation of client interactions and progress in compliance with organizational policies.

Skills

• Strong understanding of behavioral health principles and practices.

Job Type: Part-time
