Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team, providing compassionate and comprehensive patient care across various settings. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in electronic health record systems. This role offers an opportunity to work in a dynamic environment, including long-term care, acute care, hospice, and community-based services. The LPN will play a vital role in patient assessment, medication administration, and supporting treatment plans while ensuring compliance with healthcare regulations and standards such as HIPAA and NCQA.
Responsibilities
- Conduct patient assessments including vital signs, physical examinations, and health history documentation using EMR/EHR systen exprity .
- Administer medications and injections accurately following physician orders and established protocols.
- Perform specialized procedures such as IV infusion, catheterization, tube feeding, wound suturing, and sterile processing.
- Assist with diagnostic evaluations including specimen collection/processing, sonography, spinal taps, and laboratory tests.
- Manage airway care with ventilators and perform respiratory support procedures like oxygen therapy.
- Provide comprehensive patient care in various settings such as hospitals (ICU, PICU, trauma centers), outpatient clinics, nursing homes, assisted living facilities, and home care environments.
- Support patients with complex needs including dementia care (Alzheimer's and memory care), behavioral health issues, infectious disease management, and acute pain management.
- Collaborate with multidisciplinary teams to develop discharge planning and case management strategies.
- Ensure compliance with infection control protocols and aseptic techniques during all procedures.
- Maintain accurate medical documentation including ICD coding (ICD-9/10), CPT coding for procedures like Botox treatments or cardiac catheterizations.
- Educate patients and families on health coaching topics such as nutrition, medication management, and chronic disease management.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN).
- Proven experience in diverse clinical settings including hospital medicine (emergency room experience, trauma centers), long-term care (geriatrics, sub-acute care), hospice or home & community care.
- Proficiency with EMR/EHR systems
- Knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding.
- Experience with specialized procedures like IV insertion/infusion, phlebotomy/venipuncture, catheter care/suturing.
- Ability to perform patient assessments accurately and efficiently under pressure.
- Strong understanding of infection control practices and HIPAA regulations.
- Excellent communication skills to work effectively with patients of all ages—including children (pediatrics) and neonatal units—and multidisciplinary healthcare teams.
- Physical ability to assist with patient mobility using equipment like Hoyer lifts or neonatal ventilators when necessary.
