About

Personal Growth Counseling Services LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our multidisciplinary behavioral health and substance use disorder team. The LPN will work under the direct supervision of the Medical Director and in close collaboration with the Clinical Managers to ensure consistent, high-quality care across all programs. This role includes providing hands-on nursing support for pediatric, adolescent, and adult clients, assisting with medication administration, supporting Medication-Assisted Treatment (MAT), monitoring client stability, and contributing to trauma-informed, client-centered care. The LPN plays an essential role in helping the medical and clinical teams maintain continuity, safety, and excellent outcomes.

Key Responsibilities

Clinical Support & Assessment (within LPN scope)

  • Conduct basic nursing assessments, including vitals, symptom screenings, withdrawal monitoring (COWS/CIWA), and health history updates.
  • Identify and promptly report changes in client condition to the Medical Director or supervising PMHNP.
  • Support intake triage, including walk-ins, and assist with urgent clinical needs as directed by the Medical Director or Clinical Managers.

Medication Administration & Monitoring

  • Administer prescribed medications, including oral medications and injections (including Vivitrol when delegation/training requirements are met).
  • Monitor for and document medication responses or adverse reactions; communicate findings to the Medical Director or PMHNP.
  • Support medication management appointments by preparing clients, reviewing medication lists, and ensuring accurate recordkeeping.

Support for SUD & Mental Health Treatment

  • Provide compassionate nursing support to clients experiencing mental health and co-occurring substance use disorders.
  • Assist with MAT workflows, including urine drug screens, sample collection, monitoring protocols, and documentation.
  • Reinforce provider recommendations and psychoeducation to help clients understand medication adherence, wellness strategies, and follow-up care.

Collaboration & Coordination

  • Work closely with the Medical Director, Nurse Practitioners, and Clinical Managers to support coordinated, whole-person care.
  • Communicate clinical observations, safety concerns, and treatment needs to the team through established channels.
  • Assist in coordinating referrals, follow-up appointments, client scheduling, and transitions of care.

Client & Family Support

  • Provide basic education to clients and families on medication instructions, symptom management, and wellness topics, using trauma-informed and culturally sensitive communication.
  • Promote a supportive, respectful, and recovery-oriented atmosphere for clients of all ages.

Documentation & Compliance

  • Maintain accurate and timely documentation including vitals, medication logs, progress notes, and incident reports.
  • Ensure all nursing responsibilities comply with Ohio Board of Nursing standards, HIPAA, and organizational policies.
  • Follow all established procedures for medication handling, infection control, and clinical safety.

Qualifications

Education

  • Graduate of an accredited Licensed Practical Nurse program.

Licensure/Certification

  • Active, unrestricted LPN license in the state of Ohio.
  • CPR/BLS certification required.
  • IV certification preferred.
  • Medication administration training or willingness to complete training.

Experience

  • 1–2 years of nursing experience preferred, particularly in behavioral health, primary care, or substance use disorder settings.
  • Experience with detox support or mental health treatment is a plus.

Skills

  • Strong assessment and clinical observation abilities.
  • Knowledge of medication administration and monitoring protocols.
  • Ability to build rapport with clients experiencing mental health or SUD challenges.
  • Excellent communication, documentation, and teamwork skills.
  • Ability to work effectively in a fast-paced, multidisciplinary environment.
