Responsibilities

• Coordinate and facilitate social services for patients, including assessments, care planning, and discharge planning
• Develop and maintain relationships with patients, families, and healthcare providers to ensure effective communication and cooperation
• Identify and address patients' emotional, social, and psychological needs through counseling, support groups, and other interventions
• Collaborate with other healthcare professionals to develop and implement individualized care plans
• Participate in interdisciplinary team meetings to discuss patient care and progress
• Maintain accurate and timely records of patient interactions and services provided

Requirements

• At least 1 year of experience in social services or a related field (or equivalent combination of education and experience)
• Strong understanding of social services principles and practices
• Excellent communication, interpersonal, and counseling skills
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Strong organizational and time management skills
• Ability to maintain confidentiality and handle sensitive information
