Job Overview
Medina Health and Rehabilitation is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience in various medical settings. As an LPN, you will play a vital role in providing quality care to patients, assisting with their daily needs, and ensuring their comfort and safety. Your expertise will contribute significantly to the overall well-being of our patients.

Duties

  • Provide direct patient care under the supervision of registered nurses and physicians.
  • Administer medications and treatments as prescribed, including IV infusion and injections.
  • Conduct patient assessments, monitor vital signs, and document medical histories.
  • Assist in triage and emergency situations, ensuring timely interventions.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Educate patients and families on health management, medication administration, and care procedures.
  • Maintain accurate medical records and documentation in compliance with HIPAA regulations.
  • Support patients with activities of daily living (ADLs) including bathing, grooming, and feeding.
  • Participate in discharge planning and coordinate follow-up care as needed.
  • Uphold infection control protocols and ensure a safe environment for patients.

Qualifications

  • Valid LPN license in the state of practice.
  • Experience in critical care, long-term care, assisted living, or home care is preferred.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is a plus.
  • Knowledge of ICD-9/ICD-10 coding and medical terminology is beneficial.
  • Experience working with diverse populations including seniors, individuals with developmental disabilities, or those requiring memory care (Alzheimer's/Dementia).
  • Strong skills in patient assessment, medication administration, phlebotomy, and catheterization.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Excellent communication skills and a compassionate approach to patient care.
  • Basic Life Support (BLS) certification is required; additional certifications in specialized areas (e.g., pediatrics, geriatrics) are advantageous.
