Job Overview:
We are seeking a dedicated Case Manager to support individuals with Intellectual and Developmental Disabilities (IDD) by coordinating services, ensuring regulatory compliance, and promoting person-centered outcomes. The Case Manager serves as a primary advocate and point of coordination between individuals, families, providers, and community resources.
This role requires strong documentation skills, knowledge of waiver services, and the ability to manage multiple cases while ensuring quality care and compliance with state and federal guidelines.
Key Responsibilities
- Coordinate and oversee services for individuals with IDD
- Develop, monitor, and update Person-Centered Plans (PCP/ISP)
- Conduct initial and ongoing assessments and service reviews
- Ensure services comply with HCBS regulations, Medicaid waiver requirements, and state policies
- Monitor provider performance and service delivery
- Maintain accurate, timely case notes, reports, and documentation
- Advocate for individuals' rights, choices, and independence
- Collaborate with families, guardians, providers, and interdisciplinary teams
- Identify and connect individuals to community resources
- Participate in required meetings, audits, and quality assurance activities
- Respond to incidents and ensure proper reporting and follow-up
Requirements:
- Bachelor's degree in social work, Human Services, Psychology, Special Education, or a related field (required)
- Minimum of 1–2 years of experience working with individuals with IDD (preferred)
- Knowledge of Medicaid waiver services (NOW/COMP, HCBS, or equivalent) preferred
- Strong organizational, communication, and documentation skills
- Ability to manage multiple cases and deadlines
- Proficiency in electronic health record systems and Microsoft Office
- Must pass criminal background check and required screenings
- Valid driver's license and reliable transportation for community visits
Pay: $ $23.00 per hour
