Hospice Licensed Practical Nurse (LPN) Position Summary and Details

• Position Summary : Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team (IDT), provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.
• Pay : $27.72 Hourly. Full-Time Non-Exempt Position
• Schedule : 40 Hours Weekly, Monday-Friday 8:30 AM- 5:00 PM. There is an additional limited rotational on-call schedule that includes some evening, weekends, and holidays.
• Location : Would be based out of the Belleville, IL Patient Services office. Would primarily provide service to patients in the Belleville's office northern territory which includes the following Illinois counties: Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun.
• We visit our patients wherever they may located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, a valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses .67 per mile for driving personal vehicles.

Position Requirements

a. Education
1. Graduate of State-approved school of nursing.
2. Successful completion of Licensed Practice Nurse education.
b. Experience
1. One (1) year experience in hospice preferred.
c. Other Qualifications
1. Current Illinois LPN license required.
2. CPR certification preferred.
3. Excellent clinical assessment, organizational and interpersonal relationship skills.
4. A working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations is preferred.
5. Reliable means of transportation.

• Physical Requirements: Performs repetitive tasks. Physical tasks including bathing, transferring, lifting, and turning of patients in the home or home-like setting. Prolonged periods of sitting at times. Exerts up to 100 lbs. force occasionally, and/or up to 50 lbs. frequently and/or 20 lbs constantly. Requires physical flexibility of bending at knee and squat-bend. The Staff Licensed Practice Nurse manages stress appropriately, makes decisions under pressure, handles multiple priorities, works alone, and manages anger/fear/hostility/violence of other appropriately.
