Clearwater Assisted Living, LLC is a rapidly growing assisted living organization with core competencies in assisted and transitional care throughout Wisconsin. We serve adults with disabilities in the Menomonie, Chippewa Falls, and Eau Claire communities.
General Role Description:
Each Community Supported Living (CSL) program is operated by a Program Manager, two Program Supervisors and various Direct Support Professionals to care for clients. Each Program Supervisor is assigned a caseload of clients to oversee their day-to-day care and provide updates to the program. The Program Supervisor is responsible for directly managing and facilitating the care of the clients on their caseload enrolled in the CSL program, as well as overseeing and completing various administrative tasks that are instrumental in the everyday operations of the CSL program.
This position is a full-time, hourly position working between the hours of 7am - 5pm, Monday through Friday. Program Supervisors assist with problem solving, managing care updates and working with their team to provide the best care for our members. The Program Supervisor will go through Clearwater Assisted Living's comprehensive training itinerary, which will fully equip a supervisor with the skills and knowledge necessary to adequately support their DSP staff that work with our clients on a day-to-day basis. Program Supervisors act as an extension of the Program Manager.
Responsibilities:
Client Management
20-25 hours of hands-on work with clients
Assist with case management (appointments, medication, change in supports)
Provide input to Program Manager for scheduling of staff and clients
Participate in Member Centered Plan reviews
Training incoming direct support professional staff
Provide support and guidance to direct support professionals
Administration
Complete and update Mileage Logs
Review and ensure DSP staff are completing client logs
Rotating weekend on-call support
Provide guidance, direction and support to their team
Conduct client check-ins
Complete Individual Care Plan reviews monthly
Compensation:
$20.00/hr starting pay.

Requirements:
Must have a valid driver's license with an insurable driving record
Must be 18 years or older
Previous caregiving experience (Preferred)
Must be able to pass a Caregiver background check
