CARE MANAGER, Senior Services (Older Adult Services)
THIS IS A MANAGEMENT POSITION, NOT A HANDS-ON CARE ROLE.
Jewish Family & Children's Services of Northern NJ (JFCS) is a non-profit health & human services organization serving Bergen, Passaic and Hudson Counties in N.J.
This position is based in Wayne, N.J.

RESPONSIBILITIES:
- As Care Manager in our Older Adult Services department, you will provide status assessment and arrange for appropriate support by for seniors in our community, including Holocaust Survivors.
- Through an initial formal assessment, periodic home visits, and GENERAL OVERSITE of each case, the CARE MANAGER arranges for appropriate support and oversees the care plan, thus enabling seniors to remain in their homes safely. (There is mileage reimbursement for any travel.)
- At each initial face-to-face visit, assess client and determine eligibility
- Ensure that all relevant forms (and registration) are signed and submitted
- Develop a Plan of Care with client and/or caregiver
- Assist client to obtain/access both formal and informal services
- Ensure monthly contact with clients; keep service logs and progress notes in database
QUALIFICATIONS:
- You are an empathetic and responsible professional; experience with seniors a +
- Must have a bachelor's degree in a human service related field from an accredited college or university
- Two years of related experience (can include internships)
- Comfortable with databases and other computer programs (Word, Excel)
- Good assessment skills; ability to multi-task
- RUSSIAN SPEAKING A BIG PLUS
