Sapphire Nursing and Rehab at Goshen, located in Orange County, New York is actively seeking a Clinical Support Coordinator(CSC) for our Skilled Nursing Facility located in Orange, NY. The ideal candidate will have strong communication and clerical skills. The CSC will have frequent contact with residents, resident families, visitors, staff, and management personnel.

DUTIES:

Clinical Support Coordinator responsibilities:

• Assist with arranging outside clinic appointments and transportation;

• Coordinating schedule of transport aide and or family escort with resident for outside clinic appointments;

• Assist with filing medical records in resident charts;

• Responsible for scanning documents into electronic medical records;

• Basic administrative office functions; filing & data entry;

• Typing reports & correspondence as required, assisting with the admissions/discharge process.
• Relaying vital information to Nursing Staff & Medical Team;
• Provide unit staff with employee manuals & form;

• Inventories unit/office supplies & performs all other related duties;

• Other essential duties as assigned;

REQUIREMENTS:

• Minimum of a High School diploma or equivalent
• Pleasant phone manner & strong computer skills
• Solid organizational and communication skills
• Ability to multi-task in a high-pressure environment
• Should be friendly and a team worker
• Positive attitude and energy is a must.
• Dependable and willing to work
