Job Title/Position: Case Manager

Reports To: Director of Operations

JOB DESCRIPTION SUMMARY

Appropriately and accurately verify and process request for charts to be pulled for patient care, return email, and calls in a timely manner. Accurately check patient records to the appropriate chart. Release information to patients' healthcare facilities and other entities with following all laws and regulations including HIPPA.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Appropriately and pulls records for patient care.

  2. Receive or send reports from RN/LVN/DPCS or other entities to ensure patients care.

  3. Responsible to follow up with lab, X-rays in the next 48 hours. A weekly report will be sent to your Administrator with any pending orders and follow-ups every Friday at the end of the day.

  4. Responsible for calling doctors and pharmacies for new medication, refills, authorizations, physician (MD) orders, receive SOC's.

  5. Responsible for receiving incident reports, writing communication notes, history and physical (H&Ps) and any additional documents needed.

  6. Responsible to follow up with Doctors, Pharmacies, Patients, and other entities to ensure all is being done in a timely manner.

  7. Ensure that all requests for records are stamped with date/times received and logged in to the patients' files.

  8. Notify the requester when records are available.

  9. Able to retrieve files from doctors.

  10. Maintain a good working relationship within the department and other departments.

  11. Adhere to hospital requirements, policies, and standards.

  12. Provide excellent customer service.

  13. Answering the main phone line in the department.

  14. Ensures compliance with all state, federal, and Joint Commission regulatory requirements.

POSITION QUALIFICATIONS

  1. Adhere to dress code, appearance is neat and clean.

  2. Complete annual education requirements per agency policy.

  3. Maintain patient confidentiality at all times.

  4. Report to work on time and as scheduled.

  5. Maintain regulatory requirements, including all state, federal, local government regulations, and Joint Commission standards.

  6. Always represent the organization in a positive and professional manner.

  7. Comply with all organizational policies and standards regarding ethical business practices.

  8. Must possess mobility to work in a standard office setting and use standard office equipment, including a computer, vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the phone.

  9. This is primarily a sedentary office classification although standing in work areas and walking between work areas may be required.

  10. Finger dexterity is needed to access, enter, and retrieve data using a computer keyboard or calculator and to operate standard office equipment.

  11. Positions in this classification occasionally bend, stoop, kneel, reach, push and pull drawers open and close to retrieve and file information.

  12. Employees must possess the ability to lift, carry push, and pull materials and objects up to 25 pounds.

  13. Employees work in an office environment with moderate noise levels, controlled temperature conditions, and no direct exposure to hazardous physical substances.
