POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
• Case manage total care of the patient by coordinating with team members/physicians, supervising C N A activities, and participating in staff and Interdisciplinary Team meetings.
• Complete daily paperwork and charting, thoroughly and accurately according to Policy.
• Participate in on-call rotation.
• May assist with admissions

QUALIFICATIONS

• Registered Nurse in the state of Texas
• Hold a valid driver's license and have a dependable means of transportation
• 1 year of nursing experience
• Case Management experience (hospice)
• 2+ years of experience in hospice case management strongly preferred
• Proficient skill using a computer is required; Microsoft office suite experience is preferred.
