Summit Home Care and Hospice is searching for an experienced and motivated *Licensed Practical/Vocational Nurse (LPN)* to join our Hospice & Home Health teams. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

*Full-Time LPN: *Hospice & Home Health
* *Pay Rate:* $28.00 - $31.00 per Hour
* *Shift: *Dayshift, Monday – Friday (40 Hours/Week + On-Call Requirements)
* *Location: *Columbus, Ohio (Franklin County & Surrounding Counties)
* *Expected Travel:* 30 Mile Radius (Mileage Reimbursement, $0.48/Mile)
* *Sign-On Bonus: *$1,500

*Responsibilities:*
* This is a cross-functional role that will primarily support our Hospice team but may support the Home Health team as the census/caseload allows.
* The licensed practical/vocational nurse is responsible for providing direct patient care under the supervision of a registered nurse.
* Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
* Provides direct patient care as defined in the state Nurse Practice Act.
* Implement plan of care initiated by the registered nurse.
* Provides accurate and timely documentation consistent with the plan of care.
* Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
* Participates in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Spiritual Care Support) to the registered nurse and/or Director of Nursing/Clinic Managers.
* Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
* Participates in personal and professional growth and development.
* Performs other duties as assigned by the registered nurse and/or Director of Nursing.
* Cares for patients on home care and hospice as census allows.
* Weekend, Holiday and on-call rotation requirements.

*Qualifications:*
* Graduate of an accredited practical nurse or vocational nursing program.
* Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
* Possesses and maintains valid CPR certification.
* Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements.
* Recent TB test required.
* 1 year of nursing experience preferred but not required.
* Community/home health or medical/surgical experience is also preferred but not required.
* Comply with accepted professional standards and practice.
* Demonstrates good verbal and written communication and organization skills.
* Prolonged or considerable walking or standing required.
* Must be able to lift, position or transfer patients and lift supplies and equipment.
* Considerable reaching, stooping, bending, kneeling or crouching are required.
* Visual acuity and hearing are required to perform nursing skills.
* Experience is required to be eligible for the Sign-On Bonus, new graduates will not qualify.
