Job Title: Outreach Coordinator
Location: Syracuse, Utica, Watertown
Salary: $26.00 $28.00/hourly
Classification: Non-Exempt, full-time, thirty-five-hour week; occasional evenings and weekends are required for meetings, workshops, conferences, and seminars
Position Summary: The Outreach Coordinator will oversee outreach teams within assigned regions, ensuring effective service delivery and operational coordination. Responsibilities include supervising staff, managing outreach schedules, reviewing data and reports, and collaborating with other coordinators and the program manager to support services within assigned region. The coordinator will also assist with coverage as needed and perform client intakes and linkages to care and community resources. The position follows a Tuesday to Saturday schedule, including evenings and weekends.

Minimum Qualifications
Education and Experience:
Diploma or GED with at least 2 years experience in a nonprofit social service agency providing direct client services OR a comparable combination of education and experience.
A minimum of one years experience in a social service or healthcare environment delivering direct services to individuals who use drugs and members of marginalized communities, including BIPOC and LGBTQI+ individuals, is highly desirable.
Experience working with diverse populations such as people who use drugs (PWUD), individuals with justice involvement, those facing mental health challenges, or experiencing homelessness is highly preferred.
Lived experience with homelessness, substance use, justice involvement, and/or overdose is preferred. Skills and Competencies: Deep sensitivity to and understanding issues related to HIV, chronic illness, substance use, overdose, and behavioral health.
Demonstrated commitment to harm reduction principles, health equity, and the strictest standards of confidentiality (HIPAA and Article 27F). Strong dedication to upholding ACR Healths Mission, Vision, and values.
Excellent communication and documentation skills.
Ability to deliver stigma-free, culturally competent, and non-judgmental services.
Proven ability to work collaboratively with internal teams, community partners, and referral agencies. Proficiency in Microsoft Suite applications (Outlook, Word, Excel, Teams, SharePoint, PowerPoint).
Other Requirements
Valid New York State drivers license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership not required).
Willing and able to work a Tuesday to Saturday schedule, including evenings, weekends, and non-traditional business hours as needed.
Fluency in both Spanish and English is preferred.
Primary Responsibilities: Incorporate best practices regarding confidentiality into all job duties and communications, consistent with Article 27F, 42 CFR, HIPAA, ACR Healths policies, and all applicable regulations.
Oversee and coordinate outreach activities for site-specific teams, ensuring staff are supported and program deliverables are met.
Supervise or support referral processes, connecting participants to internal and external services such as harm reduction resources, screening/testing, risk reduction counseling, MOUD/MAT providers, and SDOH providers.
Maintain accurate client and service records in AIRS and other databases and prepare all required reports in a timely manner.
