We are looking to recruit a Registered Nurse who is currently licensed and registered in the state of Massachusetts to provide professional services within the scope of nursing practice standards with respect to:

  • Coordinating referrals
  • Coordinating patient admission and personnel assignments
  • Coordinating patient care with other clinicians including the patient's physician
  • Ensuring that patient needs are continually assessed and met
  • Ensuring the development, implementation, and updating of individualized patients' plans of care
  • Supervising other nurses assigned to the patient
  • Reviewing clinical documentation

A clinical manager is responsible for the coordination of care for patients assigned to the manager and must be able to perform all the duties of a Registered Nurse when necessary.

Responsibilities;

  • Understands the clinical needs of each patient. This includes regular review of progress notes and other clinical documentation with a view to ensuring that documentation meets required regulatory standards.
  • Supervises the staff assigned to the patient and coordinates care through regular communication between the clinicians.
  • Ensures all clinical visits including starts of care, supervisory visits, recertification, transfers, resumption of care, and physician orders are scheduled (where applicable) and completed satisfactorily.
  • Attends regular meetings with the Assistant Administrator to discuss patient care and any ensuing issues.
  • Works closely with the scheduling department to ensure sufficient staffing for proper patient care.
  • Is the main contact with the patient's physician and family. The clinical manager should keep abreast of all communications with the patient's physician.
  • Ensures regular contact with assigned patients to keep abreast of the patient's care. Participates in the performance improvement committees as requested to continually assess and improve the programs offered by the Agency.
  • Participates in periodic staff meetings and assists in educating nurses on required in-services, skills checks/competencies, and orientations.
  • Precepts newly hired nurses as required.
  • A Clinical Manager is available during all company's operating hours and is responsible for triaging all patients, caregivers/families, and employees' phone calls and addresses all patients and personnel needs.
  • Maintains professional licensure and actively seeks continuing education to enhance their nursing practice and patient care.
  • Perform other duties as instructed by the supervisor

A clinical manager must perform all the duties of a Registered Nurse which include:

  • Obtain intake referral information and perform the start of care visit.
  • Obtain all pertinent medical history from the client, family, or significant others at the start of care.
  • Perform all required OASIS assessments and document them as required by applicable laws and regulations.
  • Develop an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation.
  • Evaluate the client's ADL and IADL abilities and therefore need for support services such as a home health aide.
  • Determine medical necessity for other services that could enhance the positive outcome desirable for the case.
  • Develop and implement the HHA plan of care when HHA services are ordered.
  • Order other professional services as medically necessary as the client's condition warrants.
  • Perform a regular field visit as requested.

Qualifications (Preferred):
Three years of experience as a licensed professional nurse with one to two years of administrative or supervisory experience in a healthcare setting and a minimum of 1-year experience in home healthcare.

Job Type: Full-time

Pay: $93, $104,000.00 per year
