Essential Job Functions

  • Supervises DPS and Assistant House Manager (if applicable) in all aspects of their job, assisting when necessary,

in the total care of the individuals in assigned living area(s)/ community home.

  • Oversees the daily operations of their assigned living area/ community home (including but not limited to:

staffing conflicts, menu planning, grocery shopping, house meetings and resident care and support, where

applicable)

  • Ensures adequate staffing for the community home in conjunction with the Residential Department through the

use of Onshift software, maintaining the prescribed levels of supervision and budgetary restrictions.

  • Supervises and monitors the implementation of Individual Support Plans (IPPs) of the residents, ensuring the

completion and accuracy of all required documentation, including but not limited to: Care Tracker, program

goals, behavior tracking sheets, accountability checklists and incident reports.

  • Participates in the Interdisciplinary Team Process including team, IPP, Incident and Medical Meetings
  • Ensures a safe, sanitary, and family-like environment in the community home by ensuring any physical plant

concerns are quickly and correctly reported to Maintenance Department and/ or Administration

  • Oversees and assists in planning of recreational, vocational, and community inclusion activities, chaperones group

sponsored trips/social functions, participating as needed.

  • Maintains the shift report and communicates with the Assistant House Manager(s) or fellow House Manager(s) to

ensure that all relevant information is shared to ensure the health and safety of each individual entrusted to our

care as well as the ongoing daily operations of the living area/ home; conducts the shift report meeting and emails

report to the Management Team.

  • Adheres to the guidelines set forth by the Incident Management Bulletin and DGV's Incident Management (IM)

Policy to ensure the timely reporting of incidents, performs "look behinds" after an incident and takes immediate

corrective action, when necessary in order to protect the health and safety of our individuals and prevent a

reoccurrence.

  • Supports the individuals in their spiritual life by coordinating their attendance at mass, rosary or other faith

activities.

  • Ensures On-Call coverage 24 hrs. per day as assigned to ensure proper management oversight and ensure health

and safety on all three shifts.

  • Conducts Incident Investigations as assigned by the Incident Management Coordinator following the expectations

and procedures outlined in the DPW Certified Investigators curriculum; writes the Investigative Report and

provides all relevant information to the Incident Management Team.

  • Oversees the Medication Administration Process within the living area (Community Living), completing

practicum observations, Medication Administration Record (MAR) reviews; providing immediate retraining as

needed to ensure health and safety of residents.

  • Oversees the daily operations of their assigned living area/ home (including but not limited to: staffing conflicts,

menu planning, grocery shopping, house meetings and resident care and support).

Other Job Functions

  • Ensures that all employees in the community home comply with the policies and procedures of Don Guanella

Village as well as the requirements set forth by the Department of Health.

  • Ensures that each DPS and Assistant House Manager in their assigned living area(s)/ home maintains compliance

with the Training Policy inclusive of all mandatory trainings; demonstrates competence and acquires certification

in CPR/FA training as well as DPW Medication Administration Certification (Community Living), assisting to

meet the training needs of DPS and Assistant House Managers as needed.

  • Works collaboratively with other departments to communicate medical, social, and behavioral needs of

individuals served, in a timely fashion. Maintains professional communication with families, guardians, and

advocates of the residents

  • Requests and maintains Personal Needs Allowance (PNA) funds for the individuals served, and petty cash funds,

reviews purchases made and receipts for accuracy, submits receipts and change to the Client Accounts Manager in

a timely fashion.

  • Medication Administration responsibilities:
  • Practicum Observer:
  • Successfully complete and pass the DPW Medication Administration Practicum Observer course, practice

activities, examination and initial observations.

  • Maintain DPW Practicum Observer certification by completing all requirements on an annual basis within

the designated time period.

  • Oversee, monitor and supervise the administration and documentation of medication by certified DPW

Medication Administration employees.

  • Follow process and procedures to identify and report medication errors and concerns.
  • Med Certified Staff:
  • Successfully complete and pass the DPW Medication Administration course, practice activities,

examination and initial observations.

  • Maintain DPW Medication Administration certification by completing all requirements on a semi-annual

and annual basis within the designated time period.

  • Correctly and safely administer medications in the program area.
  • Adhere to procedures of documenting medication administration in the program area.
  • Follow process and procedures to identify and report medication errors.
  • Responsible for other duties as assigned by the Residential Coordinator and /or Director

Education/Experience:

  • Bachelor's degree from an accredited four-year college or university and one year related experience preferred
  • Associates degree and 5 years related experience required

Training:

  • Ability to read and write competently in English.
  • Ability to carry out instructions furnished in written, oral, or diagram form.
  • Ability to problem solve and provide solutions when challenges arise unexpectedly.
  • Working knowledge of MS Word, Excel, Outlook

Certificates and Licenses:

  • Valid (state of residence) Driver's License and driving approval from the Archdiocese of Philadelphia is required.
  • Must attend and acquire certification in the DPW Certified Investigator Training (within one year of hire date, as

training schedule permits)

Supervisory Responsibilities

  • Direct oversight of all assistant group home managers and direct care staff

Work Environment

  • While performing the duties of this job, the employee will be required to work in a variety of settings and

environments and has an occasional exposure risk to adverse weather conditions.

  • The noise level in the work environment is usually light to moderate.
  • In the event that unanticipated aggressive behavior manifests from individuals served and de-escalation techniques

fail, physical crisis intervention may be required.

  • The work hours of this job are 8 hours per day, 5 days per week but it is also expected for employee to be available

during off hours to answer work related phone calls, emails, or text messages and, if need be, be physically available

to go on site

  • The employee must frequently lift and /or move up to 50 pounds.
  • Specific vision abilities required by this job include Close vision, Distance vision, Peripheral vision, Depth perception

and Ability to adjust focus.

  • While performing the duties of this job, the employee is regularly required to sit for long periods of time; use hands to

finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to walk,

stand, and sit.

  • The employee is occasionally required to climb, stoop, kneel, crouch and taste or smell.
