About

This position support clients in their housing by providing wrap around case management supports to assist clients to maintain their housing through the lens of the Housing First philosophy.

Housing First focuses on strengthening and building the skills and abilities of the client, based on self-determined goals, which could include employment, education, social integration, improvements to health or other goals that will help to stabilize the client's situation and lead to self-sufficiency.

Activities of the role include, and are not limited to:

  • Provide wrap around case management supports to stabilize housing.
  • Ensure that data collected regarding a client is maintained in confidentiality.
  • Complete and utilize assessment tools for the clients.
  • Develop, implement and monitor an individual case management plan and crisis plan for each client on your caseload
  • Respond to conflicts, utilizing crisis intervention and prevention when required.
  • Develop relationships with clients to provide supports and housing stabilization.
  • Accompany clients to appointments based on their individual case management plans.

Qualifications:

  • Demonstrated knowledge of Housing First, Harm Reduction, Strength-based and Anti-oppressive practices
  • Exceptional communication skills – written, verbal, and interpersonal
  • Excellent organization and time management skills
  • Able to work independently
  • Mobile (with own vehicle) and comfortable delivering support in clients' homes
  • Able to trouble-shoot and resolve issues as they arise
  • Certificates in First Aid, CPR, Crisis Prevention, Motivational Interviewing, Applied Suicide Intervention Skills Training, and Intervention and Cultural-Sensitivity Training an asset

Pay: From $50,122.80 per year
